Certificate of Need

The Agency has received the following certificate of need applications for administrative review: #21-06A from Drug, Alcohol, Mental Health Counseling & Evaluation Services, Inc. for the establishment of a
10 bed Special Treatment Facility at 1111 – 2nd Floor, Fort Street Mall, Honolulu, HI, at a capital cost of $216,780; #21-07A from Drug, Alcohol, Mental Health Counseling & Evaluation Services, Inc. for the establishment of a 20 bed Special Treatment Facility at 84-1170 Farrington Highway, Unit 4B, Waianae, HI, at a capital cost of $326,780. Pursuant to Section 323D-44.5, Hawaii Revised Statutes, any interested person may request a public meeting on an application before the Agency renders its decision. Any request for a public meeting must be submitted in writing and directed to the Administrator, at the address provided below. Any such request must be received by the Agency within seven (7) days of the publication date of this notice by 4:30 p.m. HST.
