NOTICE OF HEARING AND PRE-HEARING CONFERENCE
TO: DANE S. TRAVIS, M.D.
YOU ARE NOTIFIED that pursuant to Hawaii Revised Statutes (hereafter “HRS”) Chapters 91, 92, 436B and 453, the Hawaii Medical Board will conduct a hearing and pre-hearing conference in case MED 2021-23-L. The hearing for this case will occur in accordance with the law to determine whether your physician’s license should be revoked, suspended, or otherwise disciplined, and, whether monetary fines, restitution and any other relief authorized by law should be imposed. Petition For Disciplinary Action Against License to Practice Medicine; Demand For Disclosure for this case was filed with the Office of Administrative Hearings (hereafter “OAH”) on or about October 20, 2021. The Petition allege facts and violations by you, of the provisions of HRS Chapters 436B and/or 453 and/or the related administrative rules.
The PRE-HEARING CONFERENCE will be conducted on December 2, 2021 at 9:00 a.m. The PRE-HEARING CONFERENCE will take place via telephone. Please dial 888-482-3560 at the designated time and enter the code 5878626 to join the conference.
The HEARING will be conducted on January 10, 2022 at 9:00 a.m. The HEARING will be held at Department of Commerce & Consumer Affairs, 335 Merchant Street, Honolulu, Hawaii 96813.
