VARIANCE APPLICATION NO. WW 670

Pursuant to Hawaii Revised Statutes (HRS) Section 342D-7(i), the State Department of Health (DOH) seeks written comments from interested persons regarding the following. Mr. Michael J. Carberry of the Hawaii Army National Guard has applied for a variance for the maximum of five (5) years from sections 11-62-03 and 11-62-31.1(a)(2)(D) of Hawaii Administrative Rules, “Wastewater Systems.”
The variance request is for the continued use of an individual wastewater system that is located at 4204 Diamond Head Road, Honolulu, Hawaii 96816 and TMK (1) 3-1-042: 006.
