The Agency has received a request for a public information meeting on the following certificate of need application: #18-11A for administrative review from USRC Waianae, LLC for the establishment of chronic renal dialysis services at 86-120 Farrington Hwy., Suite Nos. B211 and B212, Waianae, HI, at a capital cost of $6,642,722. The meeting will be held January 10, 2019, 10:00 a.m., at 1177 Alakea Street, Room 401, Honolulu, Hawaii. Certificate applications are reviewed according to the following criteria: relation to the state health services and facilities plan, need and accessibility, quality, cost and finances, relation to the existing health care system, and availability of resources.
